Next Steps

In order to get you started with neurotherapy and counseling (or just neurotherapy, or just counseling) we need you to complete the following forms.  You may print and fill out these forms at home, or come by the nCenter and fill them out before your appointment (please allow at least 30 minutes plus travel time).

Click the NEW CLIENT FORMS – Adult link for yourself or the NEW CLIENT FORMS – Child link if you are filling out a form for your child.  Make sure to bring the completed forms to your first appointment, or you may leave the forms with the nCenter front desk anytime sooner as well.

If you or your child plan to do counseling, you will also need to print and complete the Adult Intake Part 1 – Client History forms for yourself or the Child Intake Part 1 – Client History forms for your child before the time of your initial appointment.  Please take time and be as complete as possible when filling out the intakes.

Please arrive 30 minutes before your scheduled appointment during which time we will review your completed documents and make a copy of your photo ID, credit card (to keep on file for billing), and insurance card.  If you have any questions, please call 406-599-2492.  Thank you!

The nCenter strongly recommends calling your insurance company before your appointment. (Use the customer service number on the back of your card.) It’s important to understand your coverage and in-network provider options. Our providers include Susan Jedd, Jackie Stanley, Louise Hill, Ruth Story, and Heidi Laug who provide psychotherapy counseling, as well as Kim Johnson, our registered dietician and Dr. Lisa Murdoch our staff physician. If you have more than one insurance plan, we must know which is primary in order to submit claims. Please bring any cards with you to ensure correct claims submission.

Payment Plan and Collections

The nCenter wants to work with clients to make healthcare affordable and accessible.  It’s important that you contact the Billing staff right away if you have questions or concerns about a balance or fulfilling payment obligations.  Please call 406-599-2492 and ask for Billing.

It’s also important that you notify us with any change of address or contact information right away, even if your mail is being forwarded.  We need to be able to reach you with balance information.

The nCenter requires a $25 co-pay for clients with private pay insurance (unless stated otherwise on your insurance card) or payment in full weekly if your deductible is not met.  Co-pays and other payments are billed electronically every Friday. Our Billing staff can also put a payment plan on file for you and run your card each month on the date of your choice.
